The Registrar will need the Medical Certificate of Cause of Death
Collecting the Medical Certificate
The medical certificate will be issued by one of the following
The General Practitioner
In certain situation the doctor will issue the medical certificate of cause of death which is collected from the surgery
The Hospital Doctor
Usually the doctors who have been treating your relative/ friend will issue the medical certificate of cause of death
The Coroner
If the death has been referred to the coroner the doctor cannot issue the medical certificate. The coroner’s officer will contact the nominated next of kin of appropriate person to advise them regarding the procedures.
